This form does not take the place of any entry forms.  You are still required to complete and timely mail the entry forms in accordance with rules set forth by the Indiana State Fair or the Central Indiana Track Show Association.

* Information noted with an asterisk will only be used by myself as the Band Day Coordinator.  It will enable me to start work on the administrative database prior to receipt of the official entry forms from the Indiana State Fair in mid-July.
